4-4.8 Determining Whether a Request Provides the Requester’s Full Name and Mailing Address A request must provide the requester’s full name and mailing address. A request may not be denied for lack of provision of the requester’s full name and mailing address until the records custodian has given the requester 30 calendar days to provide his or her full name and address.
